The story of My Plan Connect Pty Ltd started in 2020 when Jodie and Michael O’Bree identified a gap in the services provided in communities for people with disabilities. Having our own loved ones with NDIS plans and knowing some of the challenges, we decided that we could provide an alternative service to the traditional care method for people with disabilities in need of quality care services.
2024 has seen us now expand into supporting the aging community, as an Aged Care provider too.
My Plan Connect have a respite home and supported living homes in our town of Leeton.
We are excited to have the opportunity to assist the vulnerable community members of Leeton to achieve their hopes and dreams. We would like to help participants lead an inclusive and ordinary lifestyle.
Our aim is to provide a caring, welcoming, and inclusive environment for our participants to feel comfortable and free to live an Ordinary life.
